FLU VACCINATIONS

The Practice is currently taking appointments for FLU VACCINATIONS.

NEW PATIENT REFERENCE GROUP

As a Practice, we are keen to set up a Patient Reference Group, and we would like to invite you to become a member. We would like to hear your views on what works, what doesn’t and what improvements you would like to see within your Practice.

We are looking to involve as many patients in the group as possible, on a voluntary basis so that we can shape and develop the services that we deliver to you. We are starting by looking for willing volunteers that are happy to be contacted periodically to get their views and maybe attend an occasional meeting. If you would like to get your families or local communities’ voice heard, please let us have your details by downloading the Expression of Interest form and returning it completed to the surgery.

DR CAMPBELL'S RETIREMENT

Dr Campbell has now retired from the Practice. All patients that are currently registered with Dr Campbell will be automatically allocated to one of the other GPs in the Practice.

However patients can if they wish request to be registered with a particular GP.

Patients will become aware of these changes on their repeat prescriptions.

NHS HEALTHCHECK

If the Practice has contacted you regarding an NHS Healthcheck, please respond to this letter by contacting the surgery on 01507 522998.

UP TO DATE CONTACT DETAILS

Please could all patients ensure that the Practice has their up to date address and contact telephone numbers so that they can be easily contacted in an emergency.

CANCELLATION OF APPOINTMENTS

In an effort to reduce waiting times please could all patients give reasonable notice when cancelling appointments.  Those patients who 'fail to attend' appointments on a regular basis may jeopardise their registration with the Practice.

Childhood Immunisations and School Leavers Vaccinations

Please keep all of your child's vaccinations up to date to prevent childhood diseases. Please contact reception to book an appointment or if you require any advice the Practice Nurses will be happy to speak to you. Clinics are held every Thursday afternoon between 2 and 4pm childhood immunisations and school leavers vaccinations at any time in the treatment room.
